Find out as much as you can about the case itself. Are you a witness in a case that involves yourself, or other individuals? Are you giving evidence to the coroner at an inquest?
Send us a copy of your report (if you have submitted one), the relevant clinical records, and any correspondence relating to the hearing. When sending information to us, please supply us with anonymised information which refers to the patient's initials and date of birth only.
