I have to write a report or statement

  1. Find out as much as you can about the purpose of the report. For example, is it for a court, police or GDC investigation?
  2. Make sure you have a copy of the relevant clinical records and any other documents that you'll need to write your report or statement.
  3. Check your report is factually accurate and detailed. Be clear about what is based on the clinical records and what is based on your recollection of events.
  4. Contact us on 0800 374 626, ideally before you submit your report or statement.
  5. Send your draft statement, the relevant clinical records, and any associated correspondence to the DDU. When sending information to us, please supply us with anonymised information which refers to the patient's initials and date of birth only.
