I have to write a report or statement

  1. Find out as much as you can about the purpose of the report. Is it for a court, police or GDC investigation?
  2. Make sure you have a copy of the relevant clinical records and any other documents that you will need to write your report or statement.
  3. Check that your report is factually accurate and detailed. Make clear what is based on the clinical records and what is based on your recollection of events.
  4. Send your draft statement, the relevant clinical records, and any associated correspondence to the DDU.
