Can I discuss an elderly patient's dental health with their relative?

You need the patient's permission to do this, and they need to have capacity. Talk to them and explain that they might benefit from family involvement, but respect their wishes if they refuse.

If the patient does not have capacity and you believe it is in their best interest to discuss their ongoing care with a family member, try and find out whether anyone has lasting power of attorney for their welfare or talk to their next of kin. Seek individual DDU advice if you are unsure.
